Health Agreement

A –The physical health of the puppy is guaranteed for 72hr after transferring to buyer, including up to date, age appropriate deworming, vaccinations, also including a thorough examination by a licensed veterinarian. Health records and vet papers will forward to buyer upon pick up day or mailed out if puppy is delivered.

B – The buyer will have 72hrs from time of transfer to have the puppy examined by their own veterinarian. If the puppy is found to be ill by a licensed veterinarian and is to be returned due to illness, a letter from the veterinarian is required deeming the puppy was unhealthy at day of sale. This includes death or illness from any health condition deemed to be present prior to the leaving of the seller’s premises.

C – This guarantee does not include minor illnesses and health issues such as colds, internal or external parasites or umbilical hernias or under or overbite or undescended testicles. However, the breeder will disclose any known illnesses or health issues prior to the sale.

D – Seller can’t assume physical health responsibility for this puppy beyond 72 hours of transition. It is the responsibility of the buyer to care for the puppy properly and keep it free from harm. Please remember that your puppy must be treated with great care and protected from danger. Exposure to any other animal, Public Park, sidewalks, grasses ect. When taking the puppy to your veterinarian be very cautious to not have the puppy come in contact with surfaces that could be contaminated by a sick dog that was brought to that facility.

E – Any Veterinarian expenses are the sole responsibility of the buyer. If the puppy needs to be returned it is at the buyers expenses.

F – The seller provides a 12 month health guarantee toward any life threatening hereditary congenital disease, if tested positive and deemed life threatening within this time frame by a veterinarian of profession the puppy can be returned and replaced with another puppy, upon availability, of equal or less value.

G – Buyer agrees to notify seller of any parasites, fleas or physical illnesses and or congenital issues, so we can evaluate the housing, parents and littermates, should there be congenital issues showing up we will not want to keep the responsible parent in our breeding program. We aim for Quality in health and temperament. Your feedback helps us evaluate if there should be any changes in our program.

Purchase/Adoption Agreement

A – Buyer must read, sign and date the Health agreement.

B – Should there accrue a need or desire to rehome the dog the owner will contact Woodlot Companions by email at info@woodlotcompanions.com, text or call 234-301-7336 or 234-301-7376 – Under no circumstances shall the dog be put in or at an animal shelter, animal rescue, pet store, a puppy mill or any unethical organization or person. The dog shall not be rehomed unless given permission by seller.

C – Agree to keep the seller updated periodically with pictures ect. at info@woodlotcompanions.com

D – Will maintain a regular schedule via self or veterinarian visit to keep dog up to date on worming, vaccinations, flea/lice treatment ect. per age and location appropriate in line with veterinarian requirement.

E – Will take responsibility for safety and well being for the life of the dog. Dog will be fed a sufficient nutritious daily diet in a monitored way, not lacking nor creating obesity. Dog will have access to sanitary fresh drinking water. Water and feed containers are kept sanitary.

F –Socialization practiced daily (human interaction) as well as other pets periodically, will be allowed to run and exercise daily outside, weather permitting. The dog will not live or be kept in a cage, crate, pet carrier ect. exceeding a 10 hour period.

G – Will not be kept in any unethical and unsanitary environment. Under no circumstances will this dog be used or abused in a Puppy Mill facility. Will not be kept on a wire/netting floor.

H – Buyer understands that all puppies exhibit certain undesirable behaviors (nipping, chewing, barking, marking, etc). Buyer is responsible for appropriate training of puppy to correctly teach acceptable behaviors, and is responsible for proper continued socialization of puppy. Buyer agrees to provide obedience training to puppy. Training may be done independently or through professional trainers. Should Buyer not be successful at obedience training or training of negative behaviors independently, Buyer agrees to seek professional help from a certified trainer should it become necessary. Behaviors that develop due to lack of proper training and socialization are not the Seller’s responsibility. Seller does not claim to be an animal behaviorist or professional trainer and recommends seeking professional help when training questions or needs arise.

I – Any disputes, mediation or legal proceedings will take place in the state of Ohio.

J – A non-refundable down payment or full payment in the amount determined by seller will reserve puppy prior to ship date. Puppy will not be shipped or released to buyer until full payment has been received in the method required by seller.

K – Have researched and understand the characteristics of the breed and the caregiving needed as the owner.

L – I am NOT under the age of 18. (ODA now requires a person to be 18 or older to buy a dog.)

M – Advertised price is pet only. Buyer agrees to not breed their dog unless discussed and agreed upon with seller. Breeding is never allowed without understanding hereditary congenital genes and other responsible breeding aspects as well. Breeding rights for all purebred AKC registered dogs will be an additional price.

N – I have read and agree to all of the afore mentioned on (Purchase/Adoption) and (Health agreement).
